FAITH FORMATION REGISTRATION K-12

This registration is for your child’s Wednesday, Faith Formation, and/or Sacrament Preparation programs only. There are other elements required as part of St. Therese, multi-focused Faith Formation, during the year including, but not limited to: Parent Nights; Family Formation (GIFT) Events; retreats for youth in Grades 6-9. Please note that Sacrament Preparation has a required parent participation and home element.

  • There are two parts to this online registration: 1)Family information and program selection; 2) Authorizations and online payment.
  • When you click ‘calculate tuition’ in part 1, your tuition owed will be displayed. Please write down this amount. You will need this number when you reach step 2—online payment.
  • Submission of forms, authorizations, and partial or full payment, complete the registration process.

  • I agree to abide by all policies and program expectations and I will make every effort to participate.
  • I understand that limited information about my family/child will be shared with those representing St. Therese who may need to contact me about program specifics (adult catechists, core team, chaperones for activities or trips, etc.).
  • I understand that my contact information will become part of the St. Therese parish database.
  • I understand that my child will be participating in Faith Formation activities on the entire St. Therese Parish campus, including but not limited to: The Education Center; Parish Center including the church, chapel, gathering space, Community Room, Youth Room; Deephaven Woods Senior Living; Outdoor spaces including the fire pit area, playground, soccer field, wooded areas
  • I understand that the United States Conference of Catholic Bishops (USCCB) requires all parishes and Catholic Schools to provide age-appropriate Personal Safety Lesson instructions annually for children and youth in Grades K-12 and that Archbishop Hebda requires all parishes to follow the requirements of the USCCB. Only personal safety curricula approved by the Archdiocese of St. Paul and Minneapolis will be used.   • I agree to indemnify the Church of St. Therese of Deephaven and the Archdiocese of Saint Paul and Minneapolis from any claims or lawsuits brought against The Church of St. Therese of Deephaven/Archdiocese of Saint Paul and Minnepolis by myself, my child or others, that arises out of any behavior by my child during Faith Formation activities. I also agree to pay reasonable attorney's fees or expenses incurred by The Church of St. Therese of Deephaven and the Archdiocese in defense of such a claim/suit.

All families who are registering will need to complete additional authorization, consent and release forms. Your registration is NOT complete until you sign these forms in part 2 of this online registration process.

Tuition

These fees help defray the expenses for curriculum, resources, supplies, building maintenance, equipment, catechist training, and staff (program support and custodial). Your enrollment, along with your continued and consistent weekend offerings make it possible for this parish to offer quality faith formation to its members. Scholarships are available for Adult catechists to defray their family's total Faith Formation fees.

Submission of this form and partial or full payment complete the registration process. No one is ever denied faith formation because of an inability to pay.

    Does not include Sacramental Programs.
  • 1st Reconciliation/1st Eucharist ~ $100 for both
    Cannot separate these Sacraments.
  • Grades 9-12 Confirmation ~ $250
  • Non-parishioner Fee ~ $100 per family not registered at St Therese
  • Adult Catechist Scholarship ~ $110
    Applies to total Family FF Fees. Non-transferable.

STARTING JUNE 2

  • Grades K-8 ~ $130/Student
  • Grades 9-12 ~ No Charge
    Supported by your Parish Stewardship dollars.
  • Family Maximum ~ No Cap
    Does not include Sacramental Programs.
  • 1st Reconciliation/1st Eucharist ~ $100 for both
    Cannot separate these Sacraments.
